The 99 question Anonymous Survey is divided into ten categories:

1) Work Environment - General; 2) Mentoring and Support; 3) Promotion; 4) Health and Safety; 5) Family; 6) Harassment/Discrimination; 7) Reporting Sexual Harassment; 8) Sexual Harassment Training; 9) Leadership; 10) Personnel Policies. Administer only the sections of the survey your Department needs information on.

This police specific survey assists management in recognizing problems before they become complaints or lawsuits and provides information that will assist departments in taking action to improve the workplace. Designed to be administered to female and male officers of all ranks, recruits in the Training Academy, and civilians. The survey should be administered by an outside organization to protect the anonymity of the respondents and to increase the survey response rate

The anonymous survey will help Chiefs answer these questions:

· ``Is sexual harassment a problem in our Department, if yes to what degree and are the perpetrators co-workers or supervisors? How is the leadership in the Department perceived regarding their efforts to prevent/stop sexual harassment?

· ``What policies and procedures do officers in the Department think would be most helpful in preventing/stopping sexual harassment? Are officers in the Department reporting sexual harassment and if not is it because they are fearful of retaliation?

· What percentage of female and/or male officers in the Depart. have mentors? How many officers don't have mentors and would like to have one? How many officers are willing to be mentors?

· What percentage of officers are planning to work towards promotion? Are there differences by gender or race? If officers are not working towards promotion, why not? How is the promotional process perceived by officers?

· What percentage of officers have problems with childcare during work hours? What's the greatest area of difficulty they have i.e. Court? What assistance by the Department would be most helpful?

· What percentage of women officers have been pregnant in the last two years? What percentage of officers have taken FMLA and were satisfied with how it was handled?

· What percentage of female officers are having problems with uniform and equipment fit in comparison to male officers? What kinds of problems are they having, if any?
